01

Identify the exact issue

Include the page URL, the exact statement or firm field at issue, the proposed correction, and a public authoritative source when available.

02

Protect confidential information

The issue tracker is public. Do not submit medical records, case facts, government identifiers, private contact information, privileged communications, or anything unsuitable for public disclosure.

03

How reports are assessed

Editorial claims are checked against cited and current official material. Firm information is compared with reliable first-party, licensing, or public-record evidence. One unreachable website does not by itself establish that a firm has closed.

04

What the audit trail records

A useful record preserves the reported detail, supporting source, review decision, date, and resulting change. Accepted firm-data corrections should flow through Injury Firm Finder so the network remains consistent.
